What is SmartHealth?

SmartHealth was designed with you in mind, to improve the health and wellness of associates and their families by offering the following free services:

• Annual physicals
• Well childcare
• Routine immunizations
• Annual gynecological exam

SmartHealth offers choice to encourage associates to take an active role in their health.

SmartHealth = Choices

SmartHealth offers the choice of comprehensive medical and geographic coverage and a wide array of medical services from two networks:

• Tier 1 for the lowest out-of-pocket cost - Any affiliated provider with the Michigan Ministries of Ascension Health for the most cost effective coverage
• Tier 2 for cost of deductible and co-insurance - Community Blue / Blue Preferred PPO network (Tier 2), with out of pocket expenses.

Because you are not required to sign up with a specific physician, changing doctors is easy. Simply select another physician listed from either the smarthealth.org Provider Director, or the Community Blue / Blue Preferred PPO directory link.

Then, just call and schedule an appointment.

Broad Service Coverage

Except for a nominal office visit co-pay, most services are covered in full with no co-pay to you when provided by a SmartHealth provider.

No claim forms are required. You can rely on your SmartHealth provider to take care of the necessary paperwork.
