Am I a Participating Provider in SmartHealth?
SmartHealth is a dual coverage health plan providing Tier 1 and Tier 2 coverage.
Tier 1 – You are a Tier 1 provider if you have signed a SmartHealth contract through one of the following Ascension Health Michigan Ministry providers:
- St. John Providence Health System (Detroit area)
- St. Mary’s of Michigan (Saginaw and Standish)
- Genesys Health System (Flint)
- Borgess Health System (Kalamazoo area)
- St. Joseph Health System (Tawas)
Tier 2 – You are a Tier 2 provider if you are affiliated with Community Blue / Blue Preferred PPO and/or Blue Cross. If you are non-participating with Blue Cross, there is no coverage, except in the case of a medical emergency.
